During the Samudra Manthan, the churning of the ocean together by gods and devils, the toxic poison ‘halahal' emerged out of the ocean. None of the gods or devils were capable of controlling it. 

Everyone prayed to Lord Shiva to handle the poison. Else the poison would have disrupted the three worlds - Heaven, Earth, Hell. To save all the living beings Lord Shiva drank the poison, which turned his neck blue, thus he got the name “ Neel Kantha”.
